We conducted a stepped-wedge cluster-randomized trial in 15 German Level I NICUs (April 2022–March 2024). VLBW infants (< 1,500 g; ICD-10-GM: P07.00, P07.01, P07.02, P07.10, P07.11) were enrolled consecutively. Exclusion criteria were lethal malformations, palliative care, and external transfers.
